Description

We are seeking a compassionate and dedicated Substance Abuse Disorder Case Manager to join our team. The ideal candidate will work directly with individuals battling substance abuse issues, providing support, guidance, and resources to help clients achieve and maintain recovery. This role involves collaborating with clients to create personalized care plans, connecting them to necessary services, and ensuring ongoing support throughout their recovery journey.

Key Responsibilities:

· Conduct comprehensive assessments of clients to determine the severity of substance abuse issues, mental health status, and any other relevant needs.

· Develop and implement individualized treatment plans in collaboration with clients, ensuring goals are specific, measurable, achievable and time bound.

· Coordinate and facilitate access to a variety of support services, including inpatient/outpatient treatment programs, counseling, medical care, housing, vocational training and legal aid.

· Provide ongoing case management services, ensuring clients remain engaged in their treatment plans and monitor progress towards recovery goals.

· Advocate for clients within healthcare and social service systems to ensure they receive necessary services and support.

· Maintain accurate and up-to-date client records, including case notes, progress reports, and treatment plans, in accordance with organization policies and regulatory requirements.

· Provide crisis intervention and support in situations where clients face immediate risks to their safety or well-being.

· Collaborate with other professionals, including social workers, counselors, healthcare providers, and law enforcement, to ensure coordinated and holistic care.

· Educate clients and their families about substance abuse disorders, recovery strategies, and resources. Available for long-term support.

· Participate in team meetings and case conferences to discuss challenging cases, share best practices, and ensure effective treatment outcomes.

· Keep abreast of current trends, resources, and best practices related to substance abuse treatment and recovery.

Qualifications:

· High school diploma or GED required; some college coursework in psychology, social work, or related fields is preferred.

· Minimum certification of a CADC I, CATC I, SUCC (equivalent or higher) is required.

· Experience in a supportive or care giving role, particularly in substance abuse, mental health or healthcare setting, is highly desirable.

· Must have working knowledge of the 12-steps and 12 core-functions, Medi-Cal experience and medical insurance experience, Utilization Review experience,

· First Aid/CPR certification is required.

· Compassionate, non-judgmental, and empathetic attitude toward individuals in recovery from addiction.

· Strong interpersonal and communication skills, with the ability to connect with individuals from diverse backgrounds.

· Ability to maintain professional boundaries while developing rapport with clients.

· Understanding of addiction, recovery, and the challenges individuals face during treatment.

· Ability to handle crisis situations and de-escalate conflicts as they arise.

· Basic computer skills and the ability to document client information accurately.

· Flexibility to work varied shifts, including evening, weekends, and holidays, as needed.

· A drug screening, background check, health screening with TB testing is required as part of our pre-employment process and all offers are contingent upon successful.

Physical Requirements:

· Ability to stand, sit, and walk for extended periods of time

· Ability to lift or move light objects

· Ability to stay awake during shift
